Care Navigator Vacancy

The Hungerford Surgery is looking for a motivated and compassionate Care Navigator to join our friendly and supportive primary care team.
This is an exciting opportunity for someone who enjoys working with people, thrives in a busy environment, and wants to play a key role in helping patients access the right care at the right time.
About the role:
Our Care Navigators are the first point of contact for many of our patients – whether by phone or at reception. Using your communication skills and training you will:
- Signpost patients to most appropriate clinician or service
- Navigate patients to community, voluntary and other NHS services
- Manage appointment bookings and enquires
- Provide high quality administrative support to our clinical and management team
- Handle sensitive information with discretion and professionalism
You will ensure patients receive timely, appropriate support while contributing to the smooth running of the practice.
About you:
We are looking for someone who:
- Has excellent communication and listening skills
- Is confident speaking to a wide range of people
- Remains calm under pressure in a busy setting
- Is organised and able to prioritise workload
- Has good IT skills (experience with TPP SystmOne is desirable but not essential)
- Demonstrate empathy, professionalism and teamwork
Previous experience in general practice, healthcare or a customer facing role would be advantageous, but full training will be provided.
